Approved Provider Status Renewal

Requirements of Renewal

To renew your Approved Provider Status, you must

• complete the Renewal Application for Approved Provider Status below;
• submit it with a list of all programs offered the previous year;
• submit a brief outline and learning objectives for each program.

Fee

The fee for renewal is $100.

Apply for Renewal

• The NDBSWE has partnered with CE Broker for all continuing education credits.
• Apply for renewal of Approved Provider Status through CE Broker (you will be taken to their website) - online form
