Purchase List of Licensed Social Workers

You may purchase a list of North Dakota licensed social workers by submitting the form and the $100 fee.  The list may be issued to you in electronic, paper, or mailing label format. Home addresses are ONLY given if the list is request for continuing education or research purposes.
